The 0-54395-3 is an Autobus Booster Assembly from Reliance Electric.  This board may have also been sold under the company names of Baldor Reliance or Asea Brown Boveri, depending on the year of manufacture.  Check your original documentation for compatibility, but typically corresponding part numbers under all these company names represent the same part. 

The 0-54395-3 has an attached front faceplate with multiple attached components.  This includes two variable dials set one on top of each other on the left-hand side of the faceplate.  Below this is a terminal strip with eight connectors.  The terminal strip allows for wiring connections to be connected to the board without the need to remove the unit from its location within a rack system.   The front faceplate is connected to the printed circuit board using screw connections.  It is marked with the board ID number and with the Reliance logo. 

Unlike many 54395 boards, the 0-54395-3 booster assembly is a four-sided board.  It is populated by resistors, capacitors, potentiometers, transistors, and integrated circuits.  Components may not be marked with reference designators but typically do carry some sort of id marking from the OEM.
